Obtaining a copy of a clinical record from a physician is not the only option available for accessing your health information.

The details recorded in your clinical file are personal information that belongs to you and to which you have access. There are different ways to review your health information, and we present them on this web page.

The Québec Health Record (QHR) is a communication platform that facilitates the secure and timely sharing of health information between authorized organizations and stakeholders. It supports their delivery of health services for the benefit of users.

Authorized providers may access and consult the information in your QHR. Therefore, the QHR cannot be accessed by all health and social services providers.

When you consult a health and social services professional (family medicine group, CLSC, emergency room, etc.), your health information is entered in the local record kept at each location where you consult. Some, not all, of the information entered in the local record is automatically shared and stored in the QHR. Only authorized persons at the location where you consulted have access to the information recorded in your local file.

You cannot access the DSQ, but the healthcare professionals and social services professionals you consult can.

You do not have to register with the QHR. Unless you refuse, authorized health information is automatically and systematically collected in the QHR. Visit the following web page for more information about the DSQ.

To obtain a copy of the file created by a physician:

You must submit a written request and provide proof of your identity at one of the following locations:

a) If you consulted the physician at a healthcare facility

Your request must be addressed to the archives department of that institution.

Please note

The following facilities are considered part of a healthcare establishment:

  • Local community service centre (CLSC);
  • Hospital centre (CH);
  • Residential and long-term care centre (CHSLD);
  • Child and Youth Protection Centre (CPEJ);
  • Rehabilitation centre (CR).
b) If you consulted them in their office, a family medicine group (GMF), or a specialized medical center (CMS)

Your request should be addressed to the privacy officer at the location where you consulted them.

c) If it is an inactive, retreated, strucked off or deceased physician

Consult the directory of physicians and residents to find out where the clinical records of his former patients are kept.

The “File transfer” section in the physician’s profile will indicate to whom and where to send a request for a copy of the file. For example:

You can obtain a copy free of charge by completing the online request form. Here is the procedure:

1. Create or log in to a secure account

  • If you do not yet have a secure account on our site, create one here: Creating an account
  • If you already have a secure account, log in here: Log in

    2. Fill out the form

In the Available Forms section of your account, select Request for Clinical Records.

3. Upload the required document

You will need to provide a copy of the health insurance card or signed consent from the patient to whom the requested clinical records belong.

Certain conditions apply to obtaining the records of a deceased person. Upon receipt of the completed form, a member of the College staff will contact the person who made the request to obtain additional information and specify the documents that must be provided in support of the request.
